Adult Diabetes Care at UMass Memorial Health

Team-Based Model of Individualized Diabetes Care

The adult care team, led by clinical chief Samir Malkani, MD, includes endocrinologists (MDs), nurse practitioners, certified diabetes care and education specialists (CDCESs), nutritionists, nurses, and patient care assistants who collaborate to customize care plans for individuals living with Type 1 and Type 2 diabetes.

Blood Glucose Management System was developed to improve blood sugar control and outcomes for all hospitalized UMass Memorial patients with diabetes through a standardized approach to inpatient care. The system integrates coordinated meal delivery, blood glucose testing and insulin administration, as well as adjusting insulin based on the carbohydrates in each meal. Staff training has strengthened collaboration among nursing, dietary services and diabetes management. The program has improved glucose management and reduced hospital stays for individuals with diabetes by 25%.
Diabetes Care Coach Program provides intensive, individualized support to individuals who are struggling to manage their blood sugar, connecting them with dedicated coaches who help with medications, daily management, nutrition, lifestyle changes, and barriers to care. Coaches work alongside the patient’s clinical team and maintain frequent contact between office visits, helping to set achievable goals and address challenges.
Diabetes in Pregnancy Program provides specialized care for women with Type 1, Type 2 or gestational diabetes. The diabetes care team works closely with maternal-fetal medicine specialists and nutritionists to adjust treatment as needs change and provide individualized guidance. This team approach provides ongoing support to help protect the health of both mother and baby throughout pregnancy and after delivery.
